MIT researchers looked at the $30–40 billion that enterprises poured into AI pilots and found that 95% of them delivered zero measurable ROI. Zero. Not disappointing returns. Not below-target returns. Nothing that could be pointed to in a board meeting and called a win.

If you're a COO or VP of Operations at a mid-market company in a regulated industry, your first instinct might be to treat this as someone else's problem. Big enterprises move slow. You're scrappier. You pick vendors carefully.

That instinct is wrong. And the reasons it's wrong are specific to your situation.

The Pilot-to-Production Gap Is Wider When Compliance Is Involved

Most enterprise AI projects die in what we call the pilot-to-production gap. The pilot works in a controlled environment. Data is clean. The team running it is motivated. The use case is narrow. And then you try to scale it — and everything that was conveniently set aside during the pilot comes rushing back.

In regulated industries, that gap is not just wide. It's filled with obstacles that don't exist in other sectors.

You have change control processes that require documented evidence before a workflow can be modified. You have validation requirements that mean an AI touching a regulated process needs formal testing, sign-off, and often a submission to a regulatory body. You have audit trails that standard SaaS AI tools weren't built to produce. You have legal review cycles that add weeks to any vendor contract.

None of this shows up in a vendor demo. None of it appears in the pilot. All of it lands on your desk when you try to make the thing permanent.

Why the ROI Math Is Broken — And Why That's Especially Dangerous Here

The MIT research flags something important: traditional ROI frameworks don't capture what AI actually costs or delivers. Organizational learning costs are invisible. Strategic positioning value is hard to quantify. Benefits accrue gradually.

That's true everywhere. But in regulated industries, there's an additional distortion. The cost of getting it wrong is asymmetric.

A failed AI pilot in a general enterprise is embarrassing and expensive. A failed AI pilot in a medical device company that touched a quality process — or in a financial services firm that automated a compliance decision — can mean a warning letter, a consent order, or personal liability for the people who signed off on it.

That asymmetry means the ROI calculation has to include risk-adjusted downside, not just expected upside. Almost no vendor will help you model that. Almost no internal AI champion wants to spend time on it. But it's the number that actually matters.

What the 67% vs. 33% Finding Actually Tells You

The research notes that vendor-led AI solutions succeed 67% of the time, versus 33% for internal builds. Operators in regulated industries often read this and conclude they should just buy, not build.

That's the wrong lesson.

Vendors succeed more often because they're selling narrow, well-scoped solutions into reasonably controlled environments. When you buy a vendor solution and try to integrate it into a regulated workflow — with your legacy ERP, your validation requirements, your change control board, your data governance policy — you are now building. You just have a vendor's UI on top.

The 67% figure applies to clean, standalone deployments. Your environment is not clean and nothing stands alone.

The Framework That Actually Works

Here's what separates the AI implementations we see succeed from the ones that stall:

Start with the workflow, not the technology. Map the process first. Identify exactly where human judgment is creating a bottleneck, where error rates are high, where rework is chronic. Then ask whether AI can address that specific failure point — not whether AI can transform the whole process.

Compliance is a design input, not a review step. Bring your quality, legal, or compliance lead into the scoping conversation before you pick a vendor. Not after. Their constraints should shape the architecture. Retrofitting compliance onto an AI workflow is how you get the worst of both worlds: slow and fragile.

Measure what you can control. Pilot-to-production progression rate. Employee adoption and fluency. Data quality improvement. Process automation coverage. These are leading indicators you can actually influence. Waiting to measure ROI at the end of the year on a system that went live four months ago is not a measurement strategy.

Build for auditability from day one. Every decision the AI makes that touches a regulated process should be logged, explainable, and retrievable. Not because a regulator is knocking on your door today. Because when they do, and they will, you want the answer to take 20 minutes, not 20 weeks.

The Bottom Line

The 95% failure rate is not a technology problem. It's a scoping, sequencing, and organizational problem. The technology is good enough. The question is whether your organization has the discipline to deploy it in a way that survives contact with regulatory reality.

If you're about to greenlight an AI pilot, ask one question before you spend a dollar: can you describe exactly what production looks like — including who owns validation, what the audit trail looks like, and how you'll handle a failure? If you can't answer that, your pilot is already on the wrong track.
